Hypnosis Agreement
I voluntarily agree to this agreement and acknowledge the potential risks, understanding that Sandra Akkermans, who will conduct the hypnosis session, is not a medical doctor, holds no degree in psychiatry, and cannot diagnose or treat any physical or mental disorder.

I choose to participate in this hypnosis session of my own free will because I wish to be here.

I understand that I am not a patient, but a participant in my own hypnosis experience.

I understand that any suggestion made during this session is part of a personal and educational motivational program and is purely informative.

I understand that my progress will depend on how I take care of myself physically, mentally, emotionally, and spiritually.

I understand that this hypnosis session is intended for educational and/or emotional purposes. It is not meant to be used as medical or psychological advice in any way. Such advice can only be provided by licensed medical or mental health professionals.

I understand that transformation is a process, and that it may take time.
DISCLAIMER: Hypnosis is not intended to diagnose, treat, or cure any specific condition. Sandra Akkermans makes no claims to offer a cure for any illness or disease. Individual results may vary. Each session is unique, and its success depends on the participant’s cooperation and trust in the process.

I am willing to be guided through relaxation exercises, visualizations, and other hypnotic techniques.

I acknowledge that these techniques are not medical but spiritual in nature. It is my responsibility to contact my healthcare provider if there are any changes in my medical condition or medication needs.

I understand that these techniques are not a substitute for conventional medical care and acknowledge that I have been advised to consult my medical professional for any new or existing medical conditions.

I understand that being hypnotized is not the same as sleeping. During a deep hypnotic trance, I may open my eyes, talk, listen, walk, laugh, and remain aware of my surroundings. I may even open my eyes and claim that “it’s not working.” However, when I surrender to these techniques and allow myself to feel and notice what arises as Sandra Akkermans speaks, I will relax and may recall situations previously unknown to my conscious mind—whether from this life or a past one.

I understand that all change is my own responsibility. I acknowledge that all healing is self-healing, and that Sandra Akkermans facilitates this method to support me in healing myself. It is my responsibility to be open and honest and to provide accurate feedback and information that can support a positive outcome.

I understand that the session will be recorded digitally for my personal use after the session. Sandra Akkermans retains the copyright of this recording. I also understand that, due to the metaphysical nature of the session and the energy present in the space, there may be a possibility that the recording is incomplete, blank, or contains static interference.
I am of legal age and I acknowledge that I voluntarily enter into a cooperative relationship with Sandra Akkermans and willingly participate in a session combining multiple relevant hypnosis techniques. I acknowledge that there is no entitlement to financial compensation, copyright claims, or any other form of demand arising from my session with Sandra Akkermans.
I have received, read, and fully understood this document.
CAT – Treatment Agreement

The client enters into treatment with the CAT-therapist and has been informed of the fees requested by this therapist.

The client is aware of the treatment form(s) used by the CAT-therapist and of the treatment plan, recognizing that such a plan is an estimate.

The CAT-therapist commits to referring the client to another CAT-therapist or medical doctor if the treatment proves inadequate or cannot continue due to illness, death, or other incapacity.

Termination of treatment may occur at any time by mutual consent.

The client may unilaterally terminate the treatment if they no longer wish or deem it necessary to continue.

If the client terminates treatment against the advice of the CAT-therapist, the client does so at their own risk and agrees to sign a statement acknowledging that they are ending treatment without the therapist’s approval.

The CAT-therapist may also unilaterally terminate the treatment if continuation cannot reasonably be expected.

The client has been informed of the complaints procedure and may submit complaints to the dispute committee associated with the CAT-therapist.

Payment is arranged by mutual agreement between the client and CAT-therapist (cash, debit, per session, etc.). The client will receive an invoice once payment has been confirmed.

The client is aware that the CAT-therapist keeps a client file.

The CAT-therapist handles this file in accordance with GDPR regulations and the standards set by the CAT board.

The client may request a copy of their file.

The client may request the destruction of their file, and the CAT-therapist is legally obliged to comply unless there is a legal reason not to or if the data is essential for the care of another client (e.g., due to hereditary conditions or other medical necessity).
